Overview:
Gentiva Home Hospice is looking for a Registered Nurse (RN) to join our team in Sandwich, MA.
This position is PRN/Per Diem status Hospice or Home Health experience needed for this per diem role.
Drive to patient home or facilityValid Driver's LicenseActive Car InsuranceMileage ReimbursementReports directly to the Administrator, Executive Director or Patient Care Manager Senior and will maintain primary control and professional management of each patient.
Act as primary liaison between physician, patient/family and hospice team.
Provide direct patient care services to hospice patients and supportive care to the patient's family with a reasonable case loadManage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice careEvaluate patient/family needs and assist in the preparation and execution of the plan of care for each patientMaintain all assigned patient clinical recordsParticipate in company quality assessment and performance improvement programs About You:
Compassionate, kind and empathetic to your patients and their familiesA current RN license (in the state of requested employment or ability to obtain)1
