SCC- In-House Liaison Representative - FT 40 hours - East Sandwich/Cape Cod

Job Summary
Full time position at Spaulding Cape Cod. Under the direction of: Admitting Director/Sr. Director of Admissions Operations.
Perform preadmission patient evaluations; coordinate the referral process to ensure a smooth transition of patients to assigned SRN facility. Assure patient meets criteria for admission, is clinically appropriate and is ready to transfer to post-acute provider. Work with SRN and referring facilities to remove barriers to create a smooth and timely transition of care.
Key Responsibilities
Referral process is coordinated and managed to ensure a smooth transition of patients to PCC in accordance with the liaison process and established timeframes.
Patient assessment information is obtained from a variety of sources and data is transmitted in appropriate systems (4Next/Rehab Checklist/EPIC).
Collaborative relationships are developed and maintained with internal staff to ensure customer satisfaction.
Referral source relationships are nurtured to ensure and maintain customer satisfaction.
Challenging transfers are supported using PCC clinical and supervisory resources.
Patients are accepted, confirmed, and qualified as described in the liaison process.
Admissions adhere to PCC criteria.
Pertinent information from PCC is communicated to the current medical provider to meet patient and provider needs.
PCC referral influences are identified and communicated to the manager for proper follow up per established guidelines.
Discharge plan alternatives are considered to determine the appropriate plan to meet the patients' needs.
Payor mix results are achieved according to established targets.
Referral and admission targets are achieved.

Qualifications Experience
Education and Degree:
Current License in Massachusetts as RN, PT, OT, SLP required
Current License in Massachusetts as RN, PT, OT, SLP, LCSW; LICSW (may also be MSW, BSW), LPN, PTA, COTA required.
Current License in Massachusetts as RN, PT, OT, SLP, LCSW; LICSW (may also be MSW, BSW), LPN, PTA, COTA required; RN, LPN and/or Social Worker preferred.
Minimum 2 years home care, acute hospital, extended care facility setting experience, or medical RX/sales experience.
Knowledge of IRF
Knowledge of LTAC
Background knowledge/work with a clinically complex population.
Knowledge of SNF and SNF regulations.
Must demonstrate effective verbal and written communication skills with the English language.
Must demonstrate basic skills in math and computer science.
Knowledge of DPH/OEMS regulations.
Knowledge of IRF, LTAC, SNF, and/or home care, levels of care and requirements .
Broad knowledge of various insurances and government reimbursements is desirable.
Highly developed communications and interpersonal skills, working with diverse population.
Work independently, be self-directed and contribute as a member of a team.
Anticipates challenges and develops and implements strategies for addressing them.
High level of service delivery. Demonstrate initiative with ability to prioritize work, meet deadlines and adapt to changing situations.
Attention to detail.
Must be able to organize and prioritize effectively while working independently and adapting to changing conditions.
Strong written, verbal and electronic communication skills.
Excellent computer skills with working knowledge of Microsoft Office Suite, and the ability to learn other software applications.
Knowledge of EPIC is helpful.
Effectively solve problems while meeting demands of all parties.
Be comfortable and enjoy working in a fast paced environment.
Be able to make appropriate clinical based on information provided, independently.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
